Appointment and Insurance Information

New Patient Appointments

New patients are asked to arrive 15 minutes early to allow for completion of paperwork. Please bring your current insurance card and driver’s license. Please contact your insurance plan and verify our providers are in network for your plan prior to your visit. You will be required to pay your copay at time of check in. Self pay patients will be expected to pay in full at time of checkout. You may ask for an estimate at the time of scheduling the appointment.

General Appointment Information

When scheduling your appointment please choose a time you do not anticipate being on your cycle. Annual visits are scheduled for 20 minutes and are devoted to preventive care. We ask that you schedule a separate visit to address any concerns and problems so that proper time and resources are devoted to your evaluation. Problem visits occurring during an annual exam will be charged an additional office visit for that extra evaluation and will be applied to your copay and deductibles by most insurance plans. During your examination the provider may find it necessary to order lab tests including pap smears. Depending on your insurance you may be billed separately for these tests by our outside reference lab.  We do not bill LabCard. Medicare allows one annual exam and one bone densitometry every two (2) years.

Cancellation and No Show Policy

Cancellations with less than 24 hour notice are disruptive to the schedule and limit our opportunity to care for other patients. Please call within 24 hours or more to reschedule or cancel whenever possible. Patients with three no show appointments will be terminated from our practice.

Financial Information

There are so many different insurance plans even within the same company that it is impossible for us to know benefits for all plans. It is your responsibility to verify with your insurance if we are in network and the benefit coverage for services. Your insurance is a contract between you and your insurance company. You are responsible for charges regardless of what your insurance pays. We accept most Mercy Health insurance plans in the Springfield office. We accept Cox insurance plans in the Branson office. We currently are not accepting new Medicaid patients without a physician referral. Established Medicaid patients need to have been seen within the past 18 months. We do not bill LabCard. We ask that prior to your appointment or visit you pay outstanding balances for coinsurance, deductible and non-covered services by your insurance in addition to the current copay. You will receive appointment and balance due reminders by phone call and/or email prior to your visit. We accept Master Card and Visa, checks and cash. Returned checks will be subject to a $25.00 service fee. We welcome the opportunity to discuss any aspect of our financial policy. Please feel free to contact our billing department.
